Accept Registration Terms and Conditions 

  By submission of this registration:

A.  I hereby give my approval for my child to participate in the 2011 Power Agility Reaction Training program (PART).  I assume all risk and hazards incidental to such participation including transportation to and from all activities, and waive, release, indemnify and agree to hold harmless PART organizers, all affiliates, sponsors, supervisors, and participants, from any claim arising out of injury to the participant named on the PART Registration form, whether the result of negligence or for any cause.

B.  I understand that all equipment provided by the organization, except for original shirt and other gifts to be named, is the property of the PART organization.

C.  I understand that as a parent or guardian of a participant, I will provide an example of sportsmanship to the participants.  I understand that the consumption of alcohol at the events is prohibited.

D.  I understand that the registration fee for my child is free.

E.  I authorize the coach, physician, or hospital to seek or provide necessary medical treatment to my child.

I CERTIFY THAT I UNDERSTAND ALL OF THE REGISTRATION INFORMATION, WILL ABIDE BY THE RULES, AND THAT ALL THE INFORMATION I HAVE PROVIDED ON THE REGISTRATION FORM IS ACCURATE.
